1. To participate, complete the survey form with your name, address and phone number and mail to Naturopathic Doctor News and Review, PO Box 2968, Scottsdale, AZ 85252 or log on to www.ndnr.com and complete the survey form for a chance to qualify, Dec 5th through Jan 16th, 20154.
2. The contest will be conducted Dec 5th through Jan 16th, 2015. Deadline for entries is Jan 16th, 2015. Grand prize drawing will be held on Friday, Jan 23th at the Naturopathic Doctor News and Review corporate office.
3. You must be 21 years of age or older to win and a resident of the United States or Canada. No purchase necessary. One entry per person is allowed. You need not be present to win. Void where prohibited.
4. The contest is open to licensed naturopathic doctor who have graduated from a CNME- accredited naturopathic graduate program with the exception of employees of Naturopathic Doctor News and Review, Metagenics, their families, agencies and representatives.
5. All taxes relative to the prize are the sole responsibility of the winner. No prize substitutions, assignments or transfers. No cash equivalents.
6. Information provided by participants in this survey will be shared by both Naturopathic Doctor News and Review and Metagenics. By submitting your entry you agree to these terms.
7. By participating, all entrants acknowledge and agree that they have entered the contest of their own free will, that the full rules and details of the contest have been made available to them in writing and they therefore understand and agree that neither the Naturopathic Doctor News & Review, Metagenics, their agents, affiliates, sponsors, representatives or employees have any liability with respect to any damages out of acceptance and use of a prize. By entering this promotion, participants agree to be bound by these rules.
