Daraxonrasib (RMC-6236) has emerged as one of the most significant advances in pancreatic cancer therapeutics in recent years. In the phase 3 RASolute-302 trial, the agent demonstrated substantial improvements in overall survival and progression-free survival in previously treated metastatic pancreatic ductal adenocarcinoma (PDAC). This article reviews the biological rationale for pan-RAS inhibition, the mechanism of action of daraxonrasib, key clinical outcomes, toxicity considerations, pharmacokinetic interaction concerns, and the implications for integrative and naturopathic oncology. Particular emphasis is placed on evidence-informed supportive care, symptom management, nutrition, exercise oncology, and drug–botanical interaction awareness.

Pancreatic ductal adenocarcinoma remains among the most lethal malignancies worldwide. Despite advances in surgery, chemotherapy, radiation therapy, and molecular oncology, long-term outcomes remain poor for many patients. The oncology community responded with unusual enthusiasm to the presentation of daraxonrasib data at ASCO 2026 because the results represented a meaningful improvement in outcomes in a disease long associated with therapeutic frustration.

important questions. What is the mechanism of action? What toxicities are expected? How should supportive care be approached? Which supplements may require caution? And how can NDs contribute constructively to multidisciplinary cancer care?

Why KRAS Matters

RAS proteins function as critical signaling nodes regulating cellular growth, survival,
proliferation, and differentiation. KRAS mutations are present in the vast majority of pancreatic adenocarcinomas. Historically, KRAS was considered difficult to target because of its molecular structure and signaling characteristics. Adverse Effects and Clinical Monitoring

Commonly reported adverse effects include rash, stomatitis or mucositis, diarrhea, nausea, vomiting, fatigue, and paronychia. Importantly, many adverse effects appear manageable through monitoring, supportive care, dose adjustments, and multidisciplinary management. 

Naturopathic physicians involved in oncology care should understand the distinction between supportive care and anti-cancer treatment claims.   • Exercise Oncology: Growing evidence supports the role of appropriately prescribed exercise in reducing fatigue, improving physical function, and enhancing quality of life in cancer patients.
  • Sleep and Circadian Health: Sleep disturbance is common during cancer treatment. Assessment of sleep quality, stress burden, and circadian disruption may improve overall wellbeing.
  • Psychological Support: Anxiety, uncertainty, and emotional distress are common in advanced cancer. Mind-body approaches, counseling, support groups, and stress-management strategies may provide meaningful benefits.
  • Acupuncture: Evidence suggests acupuncture may help selected symptoms including pain, nausea, neuropathy, and treatment-related symptom burden.

Drug–Botanical and Drug–Nutrient Considerations

Emerging pharmacokinetic data suggest that daraxonrasib is influenced by CYP3A4 metabolism, ABCB1 (P-glycoprotein) transport, and OATP transport systems. This creates important considerations for integrative practitioners. 

Potential areas requiring caution include high-dose curcumin products, berberine, concentrated green tea extracts, quercetin, CBD-rich products, THC-containing products, and other compounds with known effects on CYP3A4, P-gp, or transporter activity. 

At present, definitive human interaction studies remain limited. Accordingly, clinicians should avoid assumptions of safety and coordinate with oncology teams when combining supplements with daraxonrasib. 

Lower-risk supportive strategies may include nutrition counseling, exercise interventions, omega-3 fatty acids where appropriate, vitamin D assessment, symptom-directed supportive care, and non-pharmacologic quality-of-life interventions.

Implications for Prescriptive Naturopathic Physicians

In jurisdictions where naturopathic physicians participate more extensively in collaborative oncology care, familiarity with targeted therapies is increasingly important. Understanding mechanisms, adverse effects, laboratory monitoring, and interaction risks allows NDs to contribute meaningfully to patient care. 

Daraxonrasib also highlights a broader trend: oncology is becoming increasingly precise and molecularly informed. Integrative oncology must evolve in parallel by emphasizing evidence-informed, individualized supportive care rather than generalized protocols.

Toward Precision Integrative Oncology

The future of integrative oncology may involve precision supportive care. Just as targeted therapies are selected according to molecular characteristics, supportive interventions may increasingly be personalized according to symptom patterns, metabolic status, treatment exposures, microbiome characteristics, and functional needs. 

Research opportunities include exercise oncology, cancer rehabilitation, nutrition optimization, microbiome modulation, sleep medicine, psychosocial interventions, and survivorship planning integrated with modern targeted therapy.
