After 2 years of persistent right costal-margin pain, negative visceral evaluations, and unsuccessful conservative treatment, neurodynamic testing and targeted examination localized a suspected T10 radicular source with additional thoracic articular and peripheral neural involvement. A single image-guided, multi-structure platelet-based intervention was followed by substantial and durable improvement in pain and function.
